Referring a Patient we

Once logged in, click on “REFER PATIENT”.
Click on “REFER EXISTING PATIENT”. You will be able to provide tooth number, reason for referral and upload any pertinent radiographs.
Once you have provided all the information requested please click on “SUBMIT” located at the bottom left corner of the page.
